Job summary

Yellowstone Boys and Girls Ranch in Polson, Montana seeks a Targeted Case Manager to coordinate wrap-around services for youth and families. You will advocate, assess needs, refer to resources, and collaborate with current and prospective services to develop and monitor effective service plans.

Qualifications

• Bachelor’s degree in human services related field.
• A combination of experience and formal education may be considered in lieu of degree.
• Criminal / Child Abuse Clearance required by the State of Montana.

Responsibilities

• Advocate and guide youth and families to obtain needed services.
• Maintain contact with other service providers to meet youth and family needs.
• Research community services to identify options meeting youth needs.
• Collaborate with youth, family, and professionals to develop a treatment plan.
• Complete, update, and maintain required documentation.
• Submit and verify paperwork while maintaining PQI and program viability.
