Job summary

Yellowstone Boys and Girls Ranch in Montana seeks a Targeted Case Manager to oversee youth cases and coordinate wrap-around services with families and community providers. You will advocate, assess needs, and develop service plans while collaborating with the treatment team.

Position requires 21+, background check and a Bachelor’s degree in a human services field; strong written and oral communication skills and ability to work in multiple settings are essential.

Qualifications

• Bachelor’s degree in a human services related field.
• Must pass criminal record and child abuse registry background check.
• 21 years old or older.

Responsibilities

• Advocate, support, and guide youth and families to obtain services.
• Maintain contact with service providers to meet needs.
• Research community services to fit youth and family needs.
• Collaborate with youth, family, and professionals to develop treatment plans.
• Complete and maintain documentation per COA, ARM, contracts, and policy.
• Prepare progress notes and intake paperwork while ensuring PQI and financial viability.
