Responsibilities include building rapport, completing documentation, and facilitating life skills sessions, with a focus on housing stability and youth empowerment within CHF programs.

• Trauma-informed care and strengths-based approaches.

Responsibilities

• Build rapport with youth and serve as a positive role model.
• Provide individualized, youth-driven case management for unhoused or unstable youth.
• Maintain documentation in required systems and communicate with the multidisciplinary team.
• Conduct life skills and housing stability activities and coordinate with CES and HUD requirements.
• Monitor living environments, safety, and program operations, including Welcome Center duties.
