Job summary

Covenant House New York is seeking a Rights of Passage Case Manager to provide supportive services to 10–20 youth aged 16–21 at a Bronx ROP site. You will develop individual service plans, coordinate services, conduct counseling, and advocate for youth across partners.

Reporting to the Case Manager Coordinator, you’ll join a dedicated team of staff and residents, with duties including documentation, service coordination, and monitoring progress toward goals within a trauma-informed framework.

Qualifications

• Bachelor’s degree preferred or HS diploma with relevant experience.
• Strong group facilitation skills and ability to collaborate across departments.
• Trauma-informed approach when working with youth and diverse populations.
• Ability to manage multiple projects and meet deadlines.
• Excellent verbal and written communication; upbeat, respectful demeanor.

Responsibilities

• Provides integrated counseling and case management services to engage youth and promote goal achievement.
• Collaborates with youth to create an Individual Service Plan (ISP) and coordinates services.
• Meets with youth regularly to review progress toward ISP goals.
• Advocates for youth with external providers and collateral services.
• Maintains confidential records and ensures regulatory compliance.
• De-escalates crises and responds to emergencies per agency protocol.
