Job summary

Covenant House Texas is seeking a Case Manager to provide comprehensive case management for youth, utilizing a trauma-informed, strengths-based approach. You will advocate, coordinate with service providers, and help youths meet goals while maintaining thorough documentation.

Responsibilities include client intake, developing service plans, crisis management, and facilitating referrals to medical, housing, and employment resources.

Qualifications

• Requires a bachelor's degree in social or behavioral sciences or administrative field.
• Requires 2 years relevant experience working with youth or young adults or a combination of education and experience.
• Experience with databases; intermediate level experience with Outlook and Word required.

Responsibilities

• Provide case management services for youth including community resources and referrals for medical, parenting, employment, and housing needs.
• Practice the Trauma-Informed Care Model using a strengths-based approach.
• Maintain case records and documentation including service plans, progress notes, intake reports, and incident reports.
• Collaborate with community programs and agencies to advocate for client population needs and provide referrals.
• Transport clients to agencies and services as needed.
• Communicate effectively with community members, colleagues, and clients.
