Covenant House New York is seeking a Rights of Passage Case Manager to support 10-20 youth aged 16-21 in a Bronx ROP site. You will provide holistic case management, create individualized service plans, coordinate services, and monitor progress in achieving goals.

Reporting to the Case Manager Coordinator, you will work with resident advisors and a team of specialists in housing, education, mental health, and youth development. Apply to make a lasting impact in a mission-driven setting.

Qualificações

• High School or GED required with 1 year of residential youth experience preferred.
• Strong group leadership and collaboration with internal/external departments.
• Experience with resiliency, trauma-informed care, and youth development.

Responsabilidades

• Provides integrated counseling and case management services to engage youth and promote housing stability.
• Collaborates with youth to develop an ISP reflecting progress and goals.
• Meets with youth regularly to review ISP progress and goals.
• Advocates on behalf of youth with external service providers.
• Maintains confidential records and ensures regulatory compliance.
• De-escalates crises and follows agency protocols during emergencies.
