Job summary

Covenant House Florida Inc in Fort Lauderdale, FL is seeking an Intensive Case Manager to deliver comprehensive case management to youth in a residential setting. You will conduct intake, assessment, discharge planning, and follow-up using the TIP model to guide future goals.

The role requires a Bachelor's degree in human services or related field, at least 2 years of experience with adolescents, flexibility to work 8am-8pm, and willingness to transport youths as needed.

Qualifications

• Bachelor's degree in human services or related field required.
• Minimum of 2 years of experience working with adolescents with delinquency/substance abuse/behavioral health history.
• Flexibility to work a schedule between 8am-8pm Sunday through Saturday.

Responsibilities

• Provide intensive case management using the TIP model, including intake, assessment, discharge planning and follow-up.
• Supervise youth, maintain safety, de-escalate crises, and ensure appropriate judgement.
• Assist with community resources/referrals, and transport youth to appointments as needed.
• Document case records, incident reports, and data in required systems.
