Qualifications

• Bachelor's degree in social work, psychology, sociology, human services, or a closely related field.
• Experience with children and families facing challenges related to child welfare, behavioral health, or crisis intervention is preferred.
• Knowledge of community resources and service systems is desirable.

Responsibilities

• Engage youth, families, natural supports, and system partners in collaborative planning.
• Monitor service implementation and progress; identify and address service gaps.
• Coordinate re-entry planning for youth returning from out-of-home placements.
• Maintain timely documentation and case records; track outcomes and program data.
• Assist families in accessing supportive funding and resources.
