Job summary

Necco seeks a Wraparound Facilitator to lead family teams and deliver services in the Safe at Home/CSED Wraparound Program. You will maintain a caseload of 8–10 families, provide assessments and individualized planning, and oversee team meetings across in-home and community settings.

Qualifications

• Bachelor’s degree in a human services field; Master’s preferred.
• Licensed Social Worker or eligible for WV licensure preferred.
• Minimum 2 years experience with families or children with emotional/behavioral problems.
• Valid driver’s license and auto insurance coverage.
• Willingness to travel and participate in on-call rotation.
• Ability to use Microsoft Office, databases, EHR, and dashboards.

Responsibilities

• Maintain a caseload of 8–10 cases and lead the family team.
• Provide assessments, individualized family planning, and intensive case management.
• Oversee wraparound team meetings and supports across settings.
• Shift families from professional to natural/community supports over time.
• Participate in on-call rotation and document activities.
• Engage in community outreach to build team supports for families.
• Ensure family plans are followed and progress is documented for reporting.
