Job summary

Necco is seeking a Facilitator for the Safe at Home/CSED Wraparound Program in Logan, WV. You will lead the family team, provide assessments, and coordinate intensive case management to empower families in planning and decision-making.

The role requires maintaining a caseload of 8–10 cases, facilitating multidisciplinary meetings, and transitioning families toward natural supports. Travel and on-site work in diverse settings are essential.

Qualifications

• Must be 21 years of age or older.
• Bachelor’s degree required; Master’s preferred in a human services field.
• Minimum of 2 years experience with families/children with emotional or behavioral problems.
• Valid driver’s license and 100/300/100 auto insurance coverage.
• Willingness to travel and attend trainings as needed.
• Excellent organizational and written/oral communication skills.
• Successful completion of required criminal background checks.
• Ability to navigate Microsoft Office, databases, EHR systems, and dashboards.

Responsibilities

• Maintain a caseload of 8–10 cases at any time.
• Facilitate and lead the family team; provide assessments, individualized family planning, and intensive case management.
• Oversee wraparound meetings and supports; guide families toward natural/community supports.
• Track wraparound phases and ensure timelines are followed.
• Participate in on-call rotation and document activities for the team.
• Develop and manage family plans with collaboration from the facilitator, youth, family, and supports.
• Travel between locations as essential functions of the position.
