Job summary

Necco seeks a Wraparound Facilitator to support Safe at Home/CSED programs in Charleston and Elkview, WV. You will lead family teams, provide assessments, and guide intensive case management across home and community settings.

The role requires a client-centered, trauma-informed approach with quarterly documentation, collaboration with diverse stakeholders, and a commitment to program quality and outcomes within the Wraparound framework.

Qualifications

• Bachelor’s or Master’s degree in a human services related field
• Minimum 2 years experience working with families and/or children with emotional or behavioral problems
• Valid driver’s license and auto insurance coverage (100/300/100)
• Experience navigating software programs (e.g., Microsoft Office, EHR, dashboard)
• Must pass required criminal background checks

Responsibilities

• Maintain a caseload of 8-10 cases
• Facilitate and lead the family team
• Provide assessments, individualized family planning, intensive case management
• Oversee wraparound team meetings and supports
• Move families from professional to natural/community supports
• Participate in on-call rotation and document activities
• Engage in community outreach to build team supports
• Deliver services in multiple environments, including in-home and community settings
