CommuniCare+OLE is seeking aWraparound Facilitator in the Behavioral Health department to engage youth and families in the Wraparound process, form Child and Family Teams, and develop safety and individualized plans. The role emphasizes leadership, collaboration with multiple agencies, and adherence to Wraparound values and ethical standards.

Qualifikationen

• Minimum two years of experience with youth in Child Welfare and/or Juvenile Justice within behavioral health.
• Bilingual Spanish/English strongly preferred.
• Must provide proof of current driver’s license and auto insurance.
• BLS Certification required.

Aufgaben

• Conducts screening for clients and families involved with Child Welfare/Juvenile Justice referred to Wraparound Leadership.
• Provides information about process and procedures to help engagement.
• Facilitates initial strengths, needs, and cultural discovery for families in Wraparound.
• Convenes and facilitates Child and Family Team meetings with youth, family, supports, and professionals.
• Works with youth and family to develop safety plan and individualized plans.
• Utilizes assessment tools, compiles data, and reports outcomes.
• Completes client information and documentation within 48 hours.
• Maintains 65%+ productivity in direct client services.
• Works with clients to establish long- and short-term goals and monitors safety signals.
• Performs case management, crisis intervention, and collateral services as directed.
