Qualifications

• Bachelor’s or Master’s degree in related field preferred.
• Three years of care coordination experience.
• Previous supervisory experience preferred.
• Experience in behavioral health field required.

Responsibilities

• Supervise and oversee care coordination team members.
• Provide training for direct care coordinators in home and community.
• Ensure documentation meets Medicaid standards and reporting needs.
• Collaborate with staff, clients, and community partners.
• Participate in fidelity review and system of care adherence.
• Manage on-call rotation and ensure staff competency.
