The salary range is $52,609.67–$63,282.33, with a $5,000 hiring bonus and a comprehensive benefits package.

Qualifications

• Bachelor’s or Master’s degree in social work, education, counseling, psychology, or related field.
• LSW/LPC preferred.
• Three years of experience providing coordinated supportive services/care coordination.
• Experience in the behavioral health field; multi-system youth, autism, or developmental disabilities preferred.
• Knowledge of regulations for behavioral health and related services.

Responsibilities

• Supervise and oversee care coordination team members.
• Provide training for direct care coordinators in home and community settings.
• Support staff through conferences and case analysis.
• Maintain high fidelity wraparound training credential and ensure documentation quality.
• Coordinate on-call rotation and fidelity review of services.
