Integrated Services for Behavioral Health (ISBH) is hiring a Care Coordination Supervisor in Highland County, OH. The role focuses on high fidelity wraparound care coordination for children and families, supervising the team and ensuring quality services across multiple counties.

The salary range is $64,000.00–$74,957.37 with a $5,000 sign-on bonus. Requires LSW/LPC, with LISW/LPCC preferred, and at least three years in care coordination with supervisory experience.

Formación

• Bachelor’s degree in social work or counseling and LSW/LPC required; LISW/LPCC preferred.
• Three years of experience providing coordinated supportive services/care coordination.
• Previous supervisory experience preferred; experience in behavioral health with multi-system youth is a plus.
• Working knowledge of local, state, and federal regulations for behavioral health services.
• Excellent communication skills, both oral and written.

Responsabilidades

• Supervise and oversee activities of care coordination team members through team meetings and supervision.
• Provide training for direct care coordinators in home and community settings.
• Ensure adherence to high fidelity wraparound model and Medicaid standards.
• Collaborate with systems and partners to ensure high quality services and family voice and choice.
• Participate in staff recruitment, onboarding, and performance reviews.
