Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Care Coordination Supervisor in Highland County, OH. This leadership role ensures adherence to the high-fidelity wraparound model and supervises the care coordination team serving Region H with local partners.

The salary range is $64,000.00-$74,957.37, based on experience and licensure, with a $5,000 sign-on bonus. Bachelor’s in social work or counseling plus LSW/LPC is required; master’s preferred.

Qualifications

• Bachelor’s degree in social work or counseling; master’s preferred.
• LSW/LPC required; LISW/LPCC preferred.
• Three years of experience in care coordination or similar.
• Supervisory experience in behavioral health a plus.
• Knowledge of Medicaid and high-fidelity wraparound services is preferred.

Responsibilities

• Supervise care coordination team members through meetings and supervision.
• Provide training for direct care coordinators in home and community settings.
• Analyze cases and support staff in improving skills and documentation.
• Maintain adherence to high fidelity wraparound and ICC/MCC processes.
• Coordinate on-call rotation and collaborate with community partners.
