The role offers a $5,000 sign-on bonus and a salary between $64,000 and $74,957.37, with placement based on experience, education, and licensure.

Qualifications

• Bachelor’s or higher in social work or counseling required.
• LSW/LPC required; LISW/LPCC preferred.
• Minimum of 3 years in coordinated care coordination; supervisory experience preferred.

Responsibilities

• Supervise and oversee care coordination team activities and communications.
• Provide training for direct care coordinators in home and community settings.
• Coordinate and monitor ICC/MCC planning and related documentation to ensure Medicaid standards.
• Foster collaboration with staff, clients, and community partners to ensure family voice and choice.
• Oversee on-call rotation and fidelity reviews of the system of care.
