Job summary

Integrated Services for Behavioral Health in Highland County, OH seeks a Care Coordination Supervisor to lead the care coordination team and uphold high-fidelity wraparound care for children and families from diverse backgrounds. You will engage staff, clients, and partners to ensure coordinated services that respect family voice and choice.

The role offers a salary range of $52,609.67 – $63,282.33, a $5,000 hiring bonus, and a strong benefits package.

Qualifications

• Bachelor’s or Master’s degree in social work or related field.
• LSW/LPC preferred.
• Minimum three years coordinating supportive or care coordination services.
• Supervisory experience preferred in behavioral health.
• Knowledge of wraparound services and regulatory requirements.

Responsibilities

• Supervise care coordination team activities and ensure quality service.
• Provide training for direct care coordinators in home and community settings.
• Review documentation for ICC/MCC, care plans and assessments.
• Coordinate on-call rotation and fidelity monitoring.
• Collaborate with systems and partners to ensure family voice and choice.
