Job summary

Integrated Services for Behavioral Health is seeking a Care Coordination Supervisor in Highland County, OH. The role focuses on high-fidelity wraparound care coordination for children and families, with a strong emphasis on community resources and multi-system collaboration.

The position offers a salary range of $64,000.00–$74,957.37 based on experience and licensure, plus a $5,000 sign-on bonus. Supervisory duties and Medicaid documentation standards are essential components.

Qualifications

• Bachelor's degree in social work or counseling.
• Master's degree preferred.
• LSW/LPC required, LISW/LPCC preferred.
• Three years of experience in care coordination.
• Supervisory experience preferred.
• Experience in the behavioral health field.
• Knowledge of local, state, and federal regulations.

Responsibilities

• Supervise and oversee activities of care coordination team members through meetings and supervision.
• Provide training for direct care coordinators in the home and community setting.
• Provide routine training, support and assistance to staff through conferences and case analysis.
• Promote a culture of teamwork and recognition.
• Complete and maintain high fidelity wraparound training credential.
• Ensure completion of care plans, assessments and reports with Medicaid standards.
• Coordinate and oversee on-call rotation and fidelity reviews.
• Collaborate with systems to ensure family voice and choice.
• Assess crisis needs and respond appropriately.
• Participate in staff selection and hiring processes.
