Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Care Coordination Supervisor in Ohio to oversee wraparound care coordination for children and families. The role ensures high-fidelity implementation, builds strong community partnerships, and guides staff to deliver quality, culturally competent care.

The ideal candidate has LSW or LPC credentials, a bachelor’s or master’s in a related field, and at least three years coordinating services with supervisory experience.

Qualifications

• Bachelor’s or Master’s degree in a related field preferred.
• LSW/LPC credential required.
• Experience coordinating services/care coordination for children/families.

Responsibilities

• Supervise and oversee care coordination team activities, including supervision and communication.
• Provide training for direct care coordinators in home and community settings.
• Support staff through conferences, case problem analysis, and skill development.
• Promote teamwork and recognition within the team.
• Ensure documentation meets Medicaid and quality standards.
