Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordination Supervisor to lead the care coordination team across Region H in Southeast and Central Ohio. The role emphasizes high fidelity wraparound model adherence, staff training, and collaboration with community partners to deliver quality, person-centered services.

Qualifications

• Bachelor’s degree in social work or counseling; Master’s degree preferred.
• LSW/LPC required; LISW/LPCC preferred.
• Three years of experience coordinating care services.
• Previous supervisory experience preferred.
• Experience in behavioral health, preferably with multi-system youth or autism.
• Knowledge of state/federal regulations for behavioral health services.
• Familiarity with high-fidelity wraparound services (training after hire).

Responsibilities

• Supervise and oversee activities of care coordination team members through meetings, supervision, and regular communications.
• Provide training for direct care coordinators in the home and community setting.
• Provide routine training and support to staff through conferences and case analysis to improve skills.
• Promote teamwork and recognize staff excellence.
• Complete and maintain high fidelity wraparound training credential.
• Ensure documentation meets Medicaid standards and oversee care plans, assessments, and reports.
• Coordinate on-call rotation and participate in fidelity monitoring and system-of-care processes.
• Collaborate with systems and partners to ensure high-quality services with family voice and choice.
• Assess crisis needs and inform appropriate response; maintain confidentiality and compliance.
• Evaluate staff performance and participate in hiring and onboarding processes.
