Qualifications

• Bachelor’s or Master’s degree in social work, education, counseling, psychology, or related field.
• LSW/LPC licensure is required.
• Three years of experience coordinating supportive services.
• Previous supervisory experience preferred.
• Experience in behavioral health, multi-system youth, autism or developmental disabilities.
• Knowledge of state/federal regulations for behavioral health services.

Responsibilities

• Supervise and oversee activities of the care coordination team.
• Provide training for direct care coordinators in home and community settings.
• Ensure documentation meets Medicaid standards and supports care plans.
• Collaborate with staff, clients, and community partners to deliver high-quality services.
• Participate in fidelity reviews and wraparound service planning.
• Manage on-call rotation and team communications.

Education
