Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Care Coordination Supervisor to lead the care coordination team under the high-fidelity wraparound model in Guernsey/Noble County, OH. The role emphasizes a collaborative, family-centered approach and Medicaid-compliant documentation.

The ideal candidate will hold LSW/LPC credentials, have at least three years of experience in coordinated services within behavioral health, and demonstrated supervisory skills in diverse communities.

Qualifications

• Bachelor’s or Master’s degree in social work or related field (preferred).
• LSW/LPC required.
• At least 3 years of experience in coordinated supportive services/care coordination.
• Previous supervisory experience preferred.
• Experience in behavioral health and working with diverse youth/families.

Responsibilities

• Supervise and oversee care coordination team activities and supervision.
• Provide training for direct care coordinators in home and community settings.
• Support staff through conferences, case problem analysis, and skill improvement.
• Maintain documentation and ensure Medicaid standards are met.
• Coordinate on-call rotation and fidelity review activities.
• Collaborate with systems and partners to ensure high-quality services.
