Job summary

Integrated Services for Behavioral Health is seeking a Care Coordination Supervisor to oversee a care coordination team serving children and families in Guernsey/Noble County, OH.

The role requires LSW or LPC licensure, a minimum of three years in coordinated services, and prior supervisory experience in behavioral health. A $5,000 sign-on bonus is offered, with a salary range of $64,000 to $74,957.37 based on experience and licensure.

Qualifications

• Bachelor’s or higher in a related field and licensure (LSW/LPC) are required.
• Minimum of three years of experience coordinating care and providing services.
• Experience in behavioral health and working with multi-system youth or developmental disabilities preferred.

Responsibilities

• Supervise care coordination team activities and provide regular supervision.
• Deliver training for direct care coordinators in home and community settings.
• Ensure documentation meets Medicaid standards and maintains staff competency.
• Manage on-call rotation and fidelity review processes.
• Collaborate with systems and partners to ensure family voice and high-quality services.
