Integrated Services for Behavioral Health in Guernsey/Noble County, OH is seeking a Care Coordination Supervisor to oversee a care coordination team within a high-fidelity wraparound model. The role focuses on quality care for children and families, staff training, and ensuring Medicaid-compliant documentation.

The ideal candidate holds a social work or related degree, is licensed (LSW/LPC), and has several years of supervisory experience in behavioral health.

Qualifications

• Bachelor’s or master’s degree in social work, education, counseling, psychology, or related field preferred.
• LSW/LPC is required.
• Three years of experience providing coordinated supportive services/care coordination.
• Previous supervisory experience preferred.
• Experience in the behavioral health field is required.

Responsabilités

• Supervise and oversee care coordination team activities through meetings, supervision, and regular communications.
• Provide training for direct care coordinators in home and community settings.
• Offer routine training, support, and case problem analysis to staff to improve skills.
• Promote a culture of teamwork and staff recognition.
• Ensure documentation meets Medicaid standards and quality measures.
• Coordinate on-call rotation and participate in fidelity review processes.
• Collaborate with systems and partners to ensure high-quality services with family voice and choice.
