Job summary

Integrated Services for Behavioral Health is seeking a Care Coordination Supervisor in Highland County, Ohio to lead the high‑fidelity wraparound care coordination for children and families. You will supervise the team, provide training, ensure Medicaid‑compliant documentation, and coordinate services with community partners.

The role requires a social work or counseling degree with LSW/LPC (LISW/LPCC preferred) and at least three years of care coordination experience, plus strong communication

Qualifications

• Bachelor’s degree in social work or counseling and active licensure.
• Three years of care coordination experience with supervisory exposure.
• Experience in behavioral health and multi-system youth is preferred.
• Knowledge of Medicaid standards and regulatory requirements.
• Excellent written and oral communication, and strong organizational skills.
• Familiarity with EHR systems and care planning documentation.

Responsibilities

• Supervise and oversee activities of the care coordination team and meetings.
• Provide training for direct care coordinators in home and community settings.
• Ensure completion of care plans, assessments and required documentation.
• Coordinate on-call rotation and fidelity review of care processes.
• Collaborate with systems and partners to deliver high‑quality services.
• Participate in staff selection, onboarding, and performance reviews.
