Job summary

Comprehensive Healthcare in Walla Walla, WA seeks a Care Coordinator to deliver Wraparound with Intensive Services (WISe) program support for children and families. The role requires a Bachelor’s degree and Washington State Agency Affiliated Counselor Registration, with responsibilities spanning CANS assessments, care planning, and multidisciplinary collaboration.

Responsibilities include maintaining CANS certification, coordinating with families, documenting care notes, and supporting crisis

Qualifications

• Bachelor’s degree in behavioral science or related field.
• Washington State Agency Affiliated Counselor Registration.
• Maintains CANS certification throughout employment.
• Provide CANS screenings and full assessments every 90 days.

Responsibilities

• Maintain CANS certification throughout employment.
• Conduct CANS screening and full assessments.
• Coordinate with family, parents and community supports.
• Document service notes per guidelines with client-centered language.
• Develop measurable treatment goals with client and family.
• Participate in WISe team meetings and cross-system care planning.
• Provide after-hours coverage as needed.
• Ensure confidentiality and safe practice in all interactions.
