Job summary

The Y Social Impact Center at the Seattle YMCA seeks a WISe Care Coordinator to lead youth and family teams through the wraparound process. The role involves organizing and facilitating family team meetings, coordinating care across systems, and developing individualized care plans for clients with complex needs.

Reports to the WISe Program Director and serves in WA communities. This position is with the Y Social Impact Center, part of the YMCA of Greater Seattle, serving thousands of youth,

Qualifications

• Experience with Wraparound with Intensive Services (WISe) is a plus.
• Ability to lead and coordinate multi-agency teams in social services.

Responsibilities

• Facilitates WISe team meetings and guides the team through the WISe process.
• Provides intensive case management and care coordination to 10-12 clients in WISe.
• Uses a strengths-based, family-focused, culturally competent approach.
• Creates and reviews individualized care plans and monitors progress.
• Coordinates with WISe Therapists and WISe Peer Partners.
• Construits relationships with family members and community providers.
