Job summary

Akin is seeking a WISe Care Coordinator to lead the Wraparound with Intensive Services (WISe) process for trauma-impacted children and families. You will coordinate the Child and Family Team, facilitate meetings, and promote family-centered collaboration with therapists and community supports.

The role emphasizes partnering with families and natural supports, ensuring fidelity to wraparound principles, and coordinating with MCOs to access Medicaid services.

Qualifications

• 2 years of related experience with children and families in social services
• Strong communication skills (verbal and written)
• Ability to facilitate multidisciplinary teams and measure progress

Responsibilities

• Prepares for and facilitates Child and Family Team (CFT) meetings with agenda and resources
• Schedules times/locations convenient for family and team members
• Serves as central communication point for CFT process
• Ensures updated copies of the Cross-System Care Plan and CFT notes within 7 days
• Maintains team focus on progress toward goals and WISe transition
