Job summary

AKIN is seeking a WISe Care Coordinator to lead Wraparound with Intensive Services for trauma-impacted children and families in a collaborative, team-based setting. You will facilitate the Child & Family Team and actively partner with families, therapists, and community supports to promote safe, stable, and healthy outcomes.

Key duties include preparing CFT meetings, documenting progress in the EHR within 7 days, coordinating resources, and supporting families through ongoing check-ins.

Qualifications

• 2 years of related experience working or volunteering with children and families in social services or early intervention
• Knowledge of early childhood development with ability to foster relationships with parents, infants and youth
• CPR and First Aid Certifications (within first 30 days)
• A valid driver’s license and a favorable driving record, with no more than three violations within the past three years

Responsibilities

• Facilitate Child & Family Team meetings, including agenda and resources
• Coordinate and document progress in the family’s case records and EHR
• Maintain relations with families while coordinating community resources
• Support families through weekly check-ins and ensure consent and information releases are obtained
• Participate in after-hours crisis on-call coverage as needed
• Ensure compliance with policies and safeguarding guidelines
