Job summary

Kitsap Mental Health Services (KMHS) is hiring a Care Coordinator to provide comprehensive care coordination services for youth enrolled in the Wraparound with Intensive Services (WISe) program. You will work with youth and families in office, school, home, and community settings to ensure access to individualized care.

As a key member of a multidisciplinary team, you will plan, develop, and facilitate treatment, rehabilitation, and support services while maintaining collaborative,

Qualifications

• Bachelor's degree in psychology, social services, or behavioral health.
• Entry level; no prior related work experience required.
• Must drive clients in a personal vehicle; WA driver’s license is required (age 21+, 3+ years licensed).
• Agency Affiliated Counselor Registration.
• Preferred: 2+ years of relevant experience in related fields.
• Experience with co-occurring disorders, individual and group therapy, vocational services, or substance abuse treatment.

Responsibilities

• Provide care coordination services including individual, group, and family counseling, crisis intervention, and case management.
• Facilitate Child and Family Team (CFT) meetings to center family voice.
• Assist families in navigating mental health, education, legal, and social service resources.
• Develop and update Plans of Care aligned with treatment goals.
• Conduct CANS screenings and update every three months.
