Job summary

Akin is seeking a WISe Care Coordinator to coordinate Wraparound with Intensive Services for trauma-impacted children and families. You will lead the Child and Family Team, ensuring roles are respected and the process adheres to fidelity wraparound principles, in collaboration with peer partners, therapists, and community supports.

You will actively partner with families and natural supports to promote safe, stable, and healthy families, with a hybrid work arrangement that includes some evenings

Qualifications

• 2 years of related experience with children and families in social services or early intervention.
• Knowledge of early childhood development and ability to foster relationships with parents, infants, and youth.
• CPR and First Aid certifications completed within 30 days of hire.
• Valid driver's license with a clean driving record.
• Excellent verbal and written communication skills.

Responsibilities

• Prepare and facilitate Child and Family Team meetings with agenda and resources.
• Schedule meetings at times/locations convenient for families and team members.
• Serve as the central communication point for the team process.
• Ensure updated Cross-System Care Plans and 7-day CFT notes after meetings.
• Maintain team focus on progress toward WISe goals and transitions.
