Job summary

Behavioral Health Services in Yakima, WA seeks a Care Coordinator for the WISe program. You will engage high-risk youth and families, coordinate wraparound services, and guide the Child and Family Team to implement plans with fidelity to the WISe model.

You’ll complete CANS assessments, coordinate crisis planning, recruit supports, and provide case management and counseling to stabilize youth in the community. This role requires flexibility and travel as needed.

Qualifications

• Bachelor’s degree in social work, psychology, education, counseling or a related field.
• One year of experience in healthcare, social services, therapeutic, or educational settings.
• Valid driver’s license and automobile liability insurance.
• BLS certification within 45 days of employment.
• This role may involve bilingual differential compensation when ALTA score is reached.

Responsibilities

• Completes CANS assessments to determine WISe eligibility.
• Leads Child and Family Team meetings and supports wraparound processes.
• Recruits natural supports to join the CFT and stabilizes youth in the community.
• Provides case management, including transportation, food, medical, and housing needs.
• Delivers supportive counseling and develops individualized care plans.
