Job summary

Akinfamily is seeking a WISe Care Coordinator to coordinate a strengths-based, family-driven wraparound process for trauma-impacted children and families. You will lead the multidisciplinary Team through WISe phases, collaborating with peer partners, therapists, and community supports to promote safe, stable, and healthy families.

The role emphasizes partnering with families, managing confidential information, and ensuring high fidelity to wraparound principles in a flexible, hybrid work setting.

Qualifications

• Knowledge of early childhood development and family-centered practice.
• Experience coordinating multidisciplinary teams or care coordination.
• Ability to maintain confidential PHI/CII and adhere to HIPAA requirements.
• Strong verbal and written communication, with culturally responsive approach.

Responsibilities

• Facilitate Child and Family Team (CFT) meetings with agenda and materials.
• Schedule meetings at family-friendly times and locations.
• Act as central communication hub for the CFT process and documentation.
• Maintain up-to-date Cross-System Care Plans and progress notes within 7 days of meetings.
• Support families, ensure consent and information release processes.
