Job summary

Akinfamily seeks a WISe Care Coordinator to lead the Child and Family Team through the Wraparound with Intensive Services process for trauma-impacted children and families in a hybrid work setting. You will collaborate with peer partners, therapists, and community supports to promote safe, stable, and healthy families.

The role involves facilitating team meetings, coordinating resources, and ensuring fidelity to WISe principles while maintaining PHI/CII data in secure systems.

Qualifications

• Experience with children and families in social services or related settings.
• Ability to document and maintain records in EHR systems and according to policies.

Responsibilities

• Prepares for and facilitates Child and Family Team (CFT) meetings, including agenda and resources.
• Schedule times and locations convenient for family and team members.
• Serve as the central point of communication for the CFT process.
• Ensures updated copies of the Cross-System Care Plan and progress notes are shared within 7 days after meetings.
• Maintains team focus on progress toward goals and WISe transition.
