Qualifications

• Master's degree or relevant experience in nutrition, health education, health promotion, exercise science, public health or related field.
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist or Wellness/Lifestyle Coach in wellness/condition management.

Responsibilities

• Oversee a panel of members with varying health needs and develop personalized care plans.
• Coordinate with multidisciplinary team to refer to appropriate resources and monitor progress.
• Document activities in compliance with regulatory and accreditation standards.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Participate in continuous improvement initiatives to enhance wellness coaching services.
