• Experience in disease/condition management or wellness programs.
• Proficient in health education practices and counseling methods.

Responsibilities

• Maintain knowledge of evidence-based interventions across tobacco cessation, weight, sleep, stress, nutrition, physical activity, and maternal health.
• Assess health needs and create care plans, triage to resources as needed.
• Coordinate with multidisciplinary team to coordinate care and resources.
• Document activities per regulatory requirements and quality standards.
• Participate in program development and pilots to improve efficiency and outcomes.
