Job summary

Highmark Health is seeking a wellness coach with clinical nutrition expertise to oversee a panel of members, assess health needs, and design personalized care plans. The role combines outreach via phone and digital channels with collaboration across a multidisciplinary team to improve health outcomes and program efficiency.

The ideal candidate has a Master’s degree in nutrition/health and at least three years as a Registered Dietitian or equivalent wellness coaching experience, with HIPAA

Qualifications

• Master's degree in a health-related field or equivalent experience as determined by the company
• 3 years as a Registered Dietitian or equivalent wellness coach experience
• Experience in disease management or wellness program delivery

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards for lifestyle coaching
• Oversee a specified panel of members and assess health management needs
• Create care plans with goals and regular contact for re-assessment
• Demonstrate clinical outcomes and cost savings from wellness interventions
• Ensure documentation complies with policies and regulations
• Participate in workgroups to improve efficiency and program effectiveness
• Other duties as assigned
