Highmark Inc. is seeking a wellness coach with oversight of a member panel to tailor health interventions and create personalized care plans. You will engage members remotely or in person, coordinate with multidisciplinary teams, and monitor outcomes and costs to improve overall health results.

Qualifications include a master’s degree in a related field and 3+ years in a nutrition/dietitian, exercise physiology, or wellness coaching role, with strong communication and team collaboration skills.

Qualifications

• Master’s degree or equivalent in a health-related field; preferred in nutrition/education.
• 3+ years in a dietitian, exercise physiologist, or wellness coaching role.
• Knowledge of health education practices and behavior change models.

Responsabilités

• Maintain knowledge of evidence-based interventions and industry standards for lifestyle coaching.
• Oversee a panel of members and assess health management needs with appropriate interventions.
• Create care plans with goals and regular contact for reassessment.
• Demonstrate outcomes and cost savings from wellness interventions.
• Ensure activities are documented per regulatory and accreditation standards.
• Participate in workgroups to improve program efficiency and scope.
