Job summary

Highmark Inc. is seeking a wellness coach to manage a panel of members, assess health needs, and design personalized care plans.

You will coordinate with a multi-disciplinary team to deliver evidence-based interventions across nutrition, weight management, and lifestyle areas. You will engage members via telecommunication channels, support behavior change, and monitor outcomes to improve clinical and cost results.

Qualifications

• Master's degree in nutrition or related field, or equivalent experience as determined by the company
• 3+ years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment
• Preferred certifications: Tobacco Cessation, RDN, CDE, Exercise Physiology, Lactation Counselor, CHWC

Responsibilities

• Oversee a panel of members, assess health management needs and identify suitable clinical interventions.
• Develop personalized care plans, remove barriers, identify resources, and track progress toward goals.
• Engage with members via phone, digital, or virtual visits to improve health outcomes.
• Document all activities in compliance with regulatory and company standards.
• Participate in workgroups to improve wellness programs and drive program pilots.
• Refer to internal/external resources and monitor outcomes across age groups and regions
