Highmark Health is seeking a Wellness/Lifestyle Coach to manage a panel of members with diverse health needs. The role involves developing personalized care plans, coordinating multidisciplinary resources, and engaging with members via telephonic and digital channels to improve health outcomes.

The ideal candidate has a Master’s degree in Nutrition or related field and at least 3 years of experience in wellness/coaching within health-plan settings.

Qualifications

• Master's Degree in Nutrition or related field or equivalent experience as determined by company
• Minimum 3 years as a Registered Dietician, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment
• Ability to assess health management needs and develop care plans, coordinate resources and drive outcomes

Responsabilités

• Maintain knowledge of evidence-based interventions across lifestyle areas (tobacco cessation, weight management, sleep hygiene, stress, nutrition, physical activity, maternal health)
• Oversee a specified member panel, assess health needs, and triage to appropriate resources
• Create personalized care plans with goals and regular re-assessment contacts
• Demonstrate clinical outcomes and cost savings from wellness interventions
• Ensure documentation complies with regulatory and business requirements
• Participate in workgroups to improve program efficiency and member focus
• Other duties as assigned
