Job summary

Highmark Health seeks a skilled wellness coach to oversee a panel of members with diverse health needs. You will craft personalized care plans, coordinate with a multi-disciplinary team, and engage members via phone, digital, or virtual channels to drive better health outcomes.

You will maintain evidence-based practices in lifestyle coaching, monitor quality outcomes, and contribute to program development and pilots while ensuring regulatory compliance and strong cross-team collaboration.

Qualifications

• Master's degree in a related field or equivalent experience.
• Minimum 3 years experience as RD/EP/Wellness/Lifestyle Coach.

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Maintain oversight over specified panel of members by assessing health needs and triaging to resources.
• Create care plans to address identified needs, remove barriers, and coordinate resources.
• Engage members telephonically, digitally, or virtually to improve health outcomes.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure activities are documented and compliant with regulatory standards.
• Participate in workgroups to improve efficiency and member-centric services.
