Job summary

Highmark Health is seeking a Wellness Coach to oversee a panel of members with varying health statuses. You will assess needs, develop personalized care plans, and coordinate with a multidisciplinary team to improve health outcomes through tailored interventions.

Telephonic, digital, or virtual outreach will support member engagement. Responsibilities include delivering evidence-based coaching across tobacco cessation, nutrition, physical activity and maternal health, and ensuring compliant

Qualifications

• Master's degree in nutrition or related field required or equivalent experience as determined by the company

Responsibilities

• Maintain knowledge of evidence-based interventions for lifestyle coaching (tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, maternal health).
• Assess health management needs of the assigned panel and refer to appropriate resources.
• Create care plans with goals and regular contacts for re-assessment.
• Demonstrate outcomes and cost savings from wellness interventions across diverse member populations.
• Ensure documentation complies with applicable policies, regulations, and accreditation standards.
• Participate in workgroups to improve wellness coaching efficiency and member-centricity.
• Perform other duties as assigned.
