Highmark Health seeks a Wellness Coach to oversee a panel of members, assess health management needs, and design personalized care plans. You will use data and clinical judgment to identify interventions and coordinate with a multidisciplinary team to improve health outcomes.

Responsibilities include delivering evidence-based lifestyle coaching across tobacco cessation, nutrition, weight management, sleep, stress, and maternal health, and ensuring compliant documentation and program improvements.

Qualifications

• Master's degree as required or equivalent experience in a health-related field
• 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment

Responsabilités

• Assess health management needs of a member panel and develop care plans
• Deliver lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, activity, and maternal health
• Create personalized care plans, address barriers, coordinate resources, and re-assess progress
• Demonstrate clinical outcomes and cost savings from wellness interventions
• Ensure documentation complies with policies and standards
• Collaborate in workgroups to improve wellness services and programs
• Perform other duties as assigned
