Job summary

Highmark Health is seeking a results-driven Wellness/Lifestyle Coach who will oversee a member panel with diverse health needs. You will design personalized care plans, guide behavior change, coordinate with multidisciplinary teams, and monitor outcomes across populations via telephonic, digital, or virtual outreach.

Qualifications

• Master's degree in a health-related field or equivalent experience as determined by the company.
• 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.

Responsibilities

• Maintain evidence-based wellness interventions across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee a panel of members, assess health needs, create care plans with goals and regular re-assessment.
• Engage with members telephonically or digitally to improve health outcomes and coordinate care with multidisciplinary teams.
