Job summary

Highmark Inc. is seeking a wellness-focused professional to lead a panel of members with diverse health statuses. You will assess health management needs and design personalized care plans, coordinating with a multidisciplinary team to deliver targeted lifestyle interventions and achieve improved health outcomes.

The role involves telephonic and digital engagement with members, documenting care activities, and contributing to program pilots and quality improvements in wellness services.

Qualifications

• Master's Degree in Nutrition or related field is required or equivalent experience as determined by the company
• 3+ years as a Registered Dietitian or equivalent wellness coaching experience in a wellness-focused program
• Licensure and/or certification as a Registered Dietitian (RD) or equivalent per state requirements

Responsibilities

• Deliver high quality lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, and maternal health
• Develop personalized care plans and coordinate with multidisciplinary teams
• Engage members via phone, telehealth, or digital channels to improve health outcomes
• Monitor and report on quality outcomes (clinical, financial, and functional) for the assigned panel
• Document activities in accordance with applicable policies and regulatory requirements
