Job summary

Highmark Health is seeking a Wellness/Lifestyle Coach to oversee a panel of members, assess health management needs, and coach via telephonic, digital, or virtual channels to improve outcomes. You will develop personalized care plans, address barriers, and coordinate resources while monitoring quality and costs.

The role requires a Master’s degree or equivalent experience, plus 3 years in wellness coaching within a health plan or similar setting. Travel is 0–25% and the position is office-based.

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused environment

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee a specified panel of members by assessing health management needs and triaging to appropriate resources.
• Create care plans addressing identified needs with long- and short-term goals and regular follow-ups.
• Demonstrate clinical outcomes and cost savings from wellness interventions across diverse populations.
• Ensure activities are documented per business, regulatory and accreditation standards.
• Participate in workgroups to improve wellness coaching efficiency and new programs.
• Perform other duties as assigned.
