Highmark Health is seeking a wellness coach to manage a panel of members, assess health needs, and coordinate with a multidisciplinary team to tailor interventions. You will reach out to members by phone or digital channels to develop personalized care plans and address barriers to improve health outcomes.

Formación

• Master’s degree or equivalent experience in health-related field.
• Minimum 3 years as a wellness coach or related role in health programs.
• Preferred certifications such as Tobacco Cessation, Dietitian, CHWC, CDE.

Responsabilidades

• Maintain knowledge of evidence-based interventions to deliver high-quality lifestyle coaching.
• Oversee a specified member panel by assessing health needs and triaging resources.
• Create care plans addressing identified needs, barriers, and resources for progress.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure activities are documented in compliance with policies and standards.
• Participate in workgroups to improve efficiency, effectiveness and member focus.
• Other duties as assigned or requested.
