Qualifications

• Master's degree or equivalent in a relevant field per job posting.
• 3 years of wellness coaching experience in a health-plan or related setting.
• Licensing/certifications listed in the ad are preferred but not required.

Responsibilities

• Oversee a panel of members, assess health management needs, and refer to appropriate resources.
• Create care plans with short- and long-term goals and arrange regular follow-ups.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure all activities comply with regulatory and accreditation standards.
• Collaborate with multidisciplinary teams to enhance wellness coaching programs.
