Job summary

Highmark Health is seeking a wellness coach to lead health management for a member panel, using analytics and clinical judgment to tailor interventions. You will develop personalized care plans, remove barriers, and coordinate with a multidisciplinary team to improve outcomes.

The role requires a Master’s in a health-related field or equivalent experience, and at least 3 years in a wellness-focused setting.

Qualifications

• Master's degree or equivalent in a health-related field.
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.
• HIPAA/compliance awareness and ability to work within a health plan setting.

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards for lifestyle coaching (tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, maternal health).
• Assess health management needs and create care plans addressing identified needs; triage to resources as needed.
• Engage with members telephonically, digitally, or virtually to improve health outcomes; coordinate care with multidisciplinary team.
• Document all activities in compliance with regulatory requirements and company policies; monitor and report on outcomes and costs.
• Participate in workgroups to improve wellness coaching efficiency, effectiveness, and member-centric focus.
